Repeat for each service used e.g. Diabetes, COPD, CHF, CVD, Depression

Entering the Data

1. In Clinical Notes, Forms Tab Select Disease Management

2. Select appropriate Disease to open the TIM Web form (Parked forms are in Red)

3. The form is prepopulated with data where possible. Please check and correct as required.
4. Complete the form and submit (or Park if all information not available)

Make the claim

  • An electronic claim is created when the form is submitted
  • Create an invoice in My Practice to track payment for this service. This invoice will have the same invoice number as that submitted to TIM
    • This invoice does not need to be sent separately
