Use this form where a patient wants to apply for a sickness benefit, to enable Work and Income New Zealand to assess the patient’s eligibility. 

It covers the first and subsequent certificates. 

• Enter the current sickness benefit number on the top line if known.
• For subsequent certificates, simply press [Renew] to update the dates.
